Patient Responsibility: The patient (or parent/legal guardian) is responsible for services provided to them (or their child) regardless if it is covered by their insurance company or not. Payment is due on the same day services are rendered, unless other payment arranges have been made. There are several ways to pay your bill.

Billing & Insurance

If you receive treatment at Harmonious Living Chiropractic: Fitness & Wellness Center, you will receive an easy-to-understand bill; explaining fees, charges, and procedures.

In-Network: We are currently in-network with all CareFirst BlueCross BlueShield health plans.

Out-of-Network: If you choose to use our services, and we are not an “in-network” provider, you will be responsible for paying our actual fees; unless you have out-of-network benefits (in which your health insurance plan may reimburse for these services).

Auto Insurance: We accept all auto insurance plans (e.g. Personal Injury Protection (PIP); MedPay)


Workers’ Compensation: We accept all workers’ compensation insurance plans.

Cost for Care

Harmonious Living Chiropractic: Fitness & Wellness Center is dedicated to educating our patients on the financial responsibility for receiving care at our office. The “Good Faith Estimate” is personally based on your health insurance plan; and what portion you may be responsible for having a particular procedure(s). This allows you to see our actual fees and what your health insurance plan may reimburse for these services.

There are times when insurance do not cover healthcare services you need. Our office is an in-network provider with ChiroHealthUSA, we are to provide discounts on services for you and your family. By joining ChiroHealthUSA, you and your family will receive at least 30% discount on our actual fees for some services, making care affordable and accessible for any budget.

Complete payments online. Contact our office during normal business hours to request a special link to make a payment on your account.

Understanding Your Bill Statement: At each visit, a single billing statement will be generated and submitted to insurance on your behalf for processing. (If you utilize insurnace as a form of payment). If your insurance company does not cover services, the patient a bill that summarizes charges for all dates of services, including how much the patient paid, how much the insurance paid, and the balance on the account.

Submit a Request for Billing Records: If you want a copy of your billing records, download, complete, and return Request Copy of Patient Bill by fax to 1-888-375-5167 or mail to: PO Box 558, Columbia, Maryland 21045.

NOTE: Since patient billing records contain personal health information, you must dowload, complete and return the appropriate Request for Medical Records form to us along with the Billing Records form, if you are a non-patient requesting a copy of a patient medical records and/or billing records.
